Qualified Mental Health Professional -Adult (QMHP-A)
Position Description: Under the supervision of the Program Manager, the QMHP-A employee or contractor will provide direct Mental Health Skill Building Service guided by an individualized service plan. The service plan will provide structure, support, and services to adults that are diagnosed with mental health illness/ disorders. These services are provided in accordance with agency policies. The QMHP-A employee or contractor is required to have a background checks, training in behavioral intervention, CPR, first aid, and human rights pursuant to the state licensing standards as outlined in the policies and procedures of Forever Endeavor, LLC.
Position Responsibilities:
1. Provides direct, support, coaching, advice and training services to identified individuals as
described in individuals service plans;
2. Develops and updates individualized service plans;
3. Provide services according to individualized service plans;
4. Completes and submits weekly notes, monthly reports, quarterly reports, and assists in
planning of services;
5. Models and teaches problem-solving techniques;
6. Provides a variety of interventions in individual’s home and community as appropriate and
necessary. These interventions may be behavioral, social, supportive, and recreational in
nature;
7. Provides linkage to community-based services and supports; provides advocacy and
support with these services;
8. Coordinates scheduling of appointments, as needed;
9. Coordinates and provides transportation, as needed;
10. Available 24-hour crisis intervention and individual support;
11. Is responsible for proper and timely documentation of all information required by the
Department of Behavioral Health and Developmental Services (DBHDS) and by Forever
Endeavor, LLC;
12. Maintains regular and ongoing contact with referral agencies, as well as other agencies
involved in individual’s case;
13. Actively coordinates and plans discharge, and aftercare plans with referring agency;
14. Attends interagency meetings, as necessary;
15. Attends and participates in team meetings;
16. Attends and participates in orientation, in-service training, and conferences as scheduled;
17. Attends supervision as required;
18. Performs other duties assigned; and
19. Teaches and trains various skills to address the mental health skills-building needs of
individuals.
Position Qualifications:
A Bachelor’s Degree in the Human Services or related field (social work, psychology, psychiatrist rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling or other degree deemed equivalent to those described) from an accredited college / university, and with at least one year of clinical experience providing direct services to individuals with a diagnosis of mental health illness. Knowledge of training skills and techniques, knowledge of mental health services and agencies, and knowledge of mental health diagnosis.
To qualify as a QMHP-A, and to provide services for Forever Endeavor, LLC, the individual must have the designated clinical experience and must:
1. Be a physician; or
2. Have a master’s degree in psychology from an accredited college or university with at
least one year of clinical experience; or
3. Have a social work or human services bachelor’s or master’s degree, or at least a
bachelor’s in related field from an accredited college or university with at least one year of
clinical experience working with adults; or
4. Have a bachelor’s in any field, with at least 15 semester credits hours in human services
from an accredited college or university with at least three years of clinical experience
working with adults; or
5. Have a certification as a Psychiatric Rehabilitation Provider (CPRP) that is registered with
the United States Psychiatric Rehabilitation Association (USPRA); or
6. Be Virginia licensed, as a registered nurse, with at least one year of clinical experience
with adults; or
7. Be a licensed mental health professional.
Clinical experience means providing direct clinical services to individual with mental illness. It includes supervised internships, practicum, and field experience.
