Who is Eligible for service?

individuals who have a history of serious mental illness, that limits their ability to function independently in the community; and prior mental health treatment.

 

All of the following criteria must be met:


A. Have a need for individualized training in acquiring basic living skills;

B. Individuals ages 21 years old* and older and have one of the following qualifying mental health diagnosis, as a primary mental health disorder:

1. Schizophrenia or other Psychotic disorder;

2. Major Depressive disorder recurrent; or

3. Recurrent Bipolar I or Bipolar II; or

4. Another disorder that has been physician documented within the last year, which:

a. Is a serious mental illness;

b. Results in severe and recurrent disability;

c. Produces functional limitations in major life activities as documented in the individual’s medical record; and

d. Requires individualized training in order to achieve or maintain independent living in the community.

C. All individuals must require individualized training that is guided by goals to achieve or maintain self-regulation of basic living skills and medication treatment plans; and

D. Have had a prescription for an anti-psychotic, mood stabilizing, or anti-depressant medication within twelve months, unless a physician documents that such medication is medically contraindicated.

 

* If 18 to 21 years of age, individual must be in an independent living situation, or transferring into one within six months.
