Multidisciplinary Coordinated Approach


Forever Endeavor, LLC offers a multidisciplinary approach to help individuals achieve their desired goals and maintain independence in their communities. Individuals seek services for various, complex, and specific reasons. Consequently, Forever Endeavor, LLC reaches out to coordinate with various disciplines and supports to address each individuals' wide range of needs and continuum of care.

 

We offer outpatient crisis services to divert individuals from further decompensation and an often preventable increase in their level of care. We also offer a community support service, often as a “step-down” for individuals discharging from crisis and other more intensive services. This service provides support to individuals with emotional, behavioral, and psychological disorders who experience significant functional impairments in major life activities.
