Multidisciplinary Coordinated Approach
Forever Endeavor, LLC offers a multidisciplinary approach to help individuals achieve their desired goals and maintain independence in their communities. Individuals seek services for various, complex, and specific reasons. Consequently, Forever Endeavor, LLC reaches out to coordinate with various disciplines and supports to address each individuals' wide range of needs and continuum of care.
We offer a community support service, often as a “step-down” for individuals discharging from a more intensive level of care at some point in the their past. This service, Mental Health Skill-building (MHSS), provides support to individuals with emotional, behavioral, and psychological disorders who experience significant functional impairments in major life activities.
