Our Transitional Service


Crisis Stabilization (CS) may be the right option when after-crisis services have been identified but are not available and ready to admit an individual. CS is a short-term, person-centered crisis “bridge” service designed to support individuals in transition between levels of care when there is a gap in the availability of services. CS supports individuals face to face, in their natural environments, regardless of age, in response to an acute crisis, or as a diversion to a higher level of care. In addition, CS provides referrals and linkage to other community-based services based on individuals' immediate health and safety needs. Approval for this service is determined through authorization from an individual's  MCO and is based on medical necessity.

CS can help protect an individual against further decompensation, hospitalization, and other increased levels of care while waiting to be admitted to a service provider at the appropriate level of care.

What’s Involved?

· Brief therapeutic and skill-building     

  interventions,

· Risk Assessments,

· Psychoeducational/Health Literacy/

  Safety Planning/Crisis Education and 

  Prevention Plan,

· Engagement of natural supports,

· Integration of natural supports in the 

  de-escalation and stabilization of the 

  crisis, and coordination of follow-up

   services.


*Temporary housing is not a component of this service. However, linkages and coordination with housing and supports are available with this service.
